Holy Family Catholic School Athletics

This form must be submitted along with an Athletic Participation Physical form prior to the first practice of the season. 

Students participating in a school-sponsored sport must have a sports physical. The physical form can be found here: Athletic Participation Physicals.  The sports physical must be dated on or after May 1st of the current year. Information concerning practice times and games will be announced as soon as the schedule is complete. It is the parent's responsibility to provide transportation to and from practices and games.  If a student will be carpooling to a game or practice with another parent, they will need to complete and sign a carpool permission form. 

I give my son/daughter, named above, permission to participate in the Holy Family Catholic School Athletic Program. As the parent/guardian of the participant, I recognize and acknowledge that there are certain risks associated with participation in sports and I assume the full risk of any injuries, damages, or loss that might occur while participating in this program. In the event of any emergency, if I cannot be contacted, the school has my permission/authorization to transport my child and seek any medical treatment deemed necessary for my child's immediate care, at the nearest emergency room, and I will be responsible for payment of any and all medical services rendered.

I authorize Holy Family Catholic School to charge my FACTS account the applicable registration fee associated with the sport I selected above.
