Vision & Hearing Screening – Opt-Out Form

September 29 – October 2, 2026

All students in Kindergarten through 8th grade will have the opportunity to have their vision and hearing screened by our school nurse, free of charge.

If you DO NOT want your child to participate in the vision and hearing screening, you must complete and sign this Opt-Out Form.

IMPORTANT: If an Opt-Out Form is not submitted, your child will be screened.

VISION AND HEARING SCREENINGS

A vision and hearing screening will be conducted at Holy Family Catholic School from September 29–October 2, 2026, for students in Kindergarten through 8th grade.

Vision Screening:
The vision screening is designed to identify possible vision difficulties that may interfere with your child’s learning and school progress. The screening is not a substitute for a comprehensive eye examination by an Optometrist or Ophthalmologist (eye doctor), and no glasses will be prescribed as part of the school screening. Students who wear prescription glasses will not receive a vision screening, as they are already under the care of an eye doctor.

If the screening indicates that your child may need further evaluation, you may receive a letter from the school nurse recommending that your child see an eye doctor for additional assessment.

Hearing Screening:
The hearing screening is designed to identify students who may have a possible hearing loss. Hearing difficulties can affect a child’s speech and language development, learning, and social development.

If the screening indicates that your child may need further evaluation, you may receive a letter from the school nurse recommending that your child see an audiologist for additional assessment.

IMPORTANT INFORMATION FOR ABSENT STUDENTS:
The school conducts vision and hearing screenings only once each school year. If your child is absent during the screening period, they will not have another opportunity to be screened at school. Parents of students who are absent during the screening will receive a letter recommending follow-up with an appropriate healthcare provider.

 

Please note: Receiving a letter does not mean that your child has a confirmed vision or hearing problem. It means that further evaluation by a healthcare professional is recommended.
